The Undersigned hereby request the Wester Allegheny County Municipal Authority grant water service to the propperty at

Should the Authority grant the service and afford a supply of water as requested, the undersigned hereby agrees to assume all responsibility arising from the breakage or leakage of pipes or damages to meter, or theft or unlawful removal of same, and the undersigned hereby releases the Western Allegheny County Municipal Authority from any and all damage caused by water from any breakage or leakage of mains in service lines.

The undersigned, severally and jointly, also agrees to pay for the water supplied at the rates now in force or which may be established hereafter and that such water supply shall be subject to the rules, regulations and conditions which the Authority have heretofore adopted, or which may be adopted hereafter from time to time, and which are on file at the Authority office, for the preservation, regulation and protection of the water system of the Wester Allegheny County Municipal Authority.

The undersigned, severally and jointly, further agrees that he will not permit or allow acquiesce in any person, partnership or corporation to tap onto the water line. In the event that the undersigned in violation of this agreement should permit any person, as above, to tap onto said water line, the undersigned hereby agrees that the sum of One Hundred Twenty-five Dollars ($125.00) shall be the amount of the liquidated damages owing to the Water Authority.

In the event that the undersigned defaults in any payment of water rental, or the payment of the liquidated damage above referred to, or any other charge legally made against the undersigned by the Authority, then, in that event the undersigned does hereby empower any attorney of any court of record within the United States or elseware to appear for him and with or without declaration filed, confess judgment against him and in favor of the Western Allegheny County Municipal Authority, its successors or assigns for any sum or sums due, together with an attorney's commission of ten percent (10%), for collection, and release of all errors, and without stay of execution and inquisition and extension upon any levy is hereby waived, and condemnation agreed to and the exemption law now in force or which may be hereafter passed. Waiver of any default shall not affect the rights of the Western Allegheny County Municipal Authority upon any later default.

IT IS FURTHER UNDERSTOOD THAT ANY EVIDENCE OF TAMPERING WITH THE METER, METER SEAL OR SERVICE BOX WILL MEAN IMMEDIATE DISCONTINUENCE OF WATER.

It is further understood that the owner or owners herein shall be and surely for the tenant or tenants herein. It is further understood that in the event a new tenant should lease the house, apartment, or premises referred to herein, then in that event all parties must sign a new application or upon failure to do so within (30) days of notice the water service shall be discontinued.

The undersigned agrees to be bound hereby.
